Immunization Requirements for every school year!

Immunizations are required for all students attending school in the state of Georgia.

The "certificate of immunization" (Form #3231) must be completed by the health department or physician and must be marked with either "Date of Expiration" or as "Complete for School Attendance." This form should be sent to the school's Student Support Center (Christi.Lowe@gcpsk12.org) or uploaded via ParentVUE.

Effective in 2021, the Ga Dept of Health added Tdap and MCV4 the required immunizations. 
11th Graders must get the MCV4 vaccine ON/AFTER their 16th birthday, but not MORE THAN 30 days after their 16th birthday.

These immunizations can be administered by your family doctor,  The Lawrenceville Health Center (appointments: 770-904-3717)... though many pharmacies (Publix, Kroger, CVS, etc) can administer them, you would likely have to take the pharmacies documentation to the health dept or doctor so they can upload it to the correct platform that pharmacies don't have access to.
